Information for all students:

•  Letter from Mr. Glowacki

•  2008-09 School Calendar (2 pages)

•  OLSM Physicals Letter

•  AVI Lunch Information Letter

All students must submit:

•  Student Registratin Form and Payment Contract (2 pages)

•  Physical Form

•  Immunization Form

•  Consent to Treat Form (2 pages)

•  Off Campus Permission Slip

•  Parents' Honor Code Form

Student athletes must also submit:

•  MHSAA Consent Form (2 pages)

•  Athletic Emergency Form

Students who live on campus must also submit:

•  Kolbe Hall Form

PLEASE NOTE: If students are not accompanied by their parent/guardian for the physical, we must have the Physical Form, Consent To Treatment Form and the new MHSAA Medical History/Student and Parent or Guardian Consent Form completely filled out with a parent's signature in order to allow your student to have a physical.  All forms are available for download on our website.
